PROOF OF TRAVEL

Provide one of the following

  • Travel Itinerary
  • Plane Ticket
  • Applicant’s name must appear on documentation

    VALID PASSPORT

    Original Version (NOT COPY)

  • Must be valid for the next (6) six months
  • Have at least two blank visa pages back to back
  • Not be frayed, torn, separating, or altered in any other way
