Provide one of the following

Travel Itinerary

Plane Ticket

Applicant’s name must appear on documentation

Original Version (NOT COPY)

Must be valid for the next (6) six months

Have at least two blank visa pages back to back

Not be frayed, torn, separating, or altered in any other way

Take two (2) copies of your passport sized photographs. See guidelines bellow
