Provide ONE of the following:

• Original Marriage license
• Original Divorce Decree
• Original Court order showing the name change 

Note: Photocopies of documents are not accepted by the Passport Agency

The document must show the official seal of the issuing authority. All official documents will be returned to you with your amended passport.
