-*Application preparation or rewrite will be charged additional fees on site*

-Please make sure that your name is spelled exactly the same as the name appearing on the relevant ID or document. No variations are permitted.

-An original signature of the applicant on the signature line on the application form. Please make sure that your signature is on the line, and that the signature must match the signature in the applicants passport.

–Every question on the application must be answered, answer NONE where it does not apply to you. 

-Applications must be typed in BLOCK CAPITAL LETTERS; handwritten changes are not accepted.on the applicant’s passport. 

-DOUBLE SIDED APPLICATIONS ARE NOT ACCEPTED.  

-New York: Travelers who have visited Turkey in the past must provide a signed copy of an affidavit giving the exact dates and purpose(s) of every trip make to Turkey, Kazakhstan and Pakistan. The dates must match all entry/exit stamps in the traveler’s passport.
