DS-11 PASSPORT APPLICATION

PASSPORT SIZE PHOTOS
Take two (2) copies of your passport sized photographs. See guidelines bellow
PROOF OF CITIZENSHIP
Provide one of the following

The driver’s license must have been issued a minimum of 6 month ago.
PROOF OF TRAVEL
Provide one of the following
Applicant’s name must appear on documentation
Special Requirements Parent’s Consent Form DS-3053

Visit an Authorized Application Execution Office
Go to an Authorization Agent
Visit an Authorized Post Office, County Clerk Office or Clerk of Court with Passport Acceptance Agent
