PASSPORT SIZE PHOTOS
Take two (2) copies of your passport sized photographs. See guidelines bellow
PROOF OF TRAVEL
Provide one of the following

SPECIAL REQUIREMENTS: OUT OF VISA PAGES
Only If Applies To Your Situation If your passport has not yet expired and is out of visa pages you may renew it. If your passport will expire in more than a year from the time you plan to apply for a renewal you will need to provide a statement indicating why you wish to renew at this time.
